To pay by check, please submit full payment and completed registration form by mail to the IMPACT Implementation Center.

Fees

IMPACT is a team-based approach to depression treatment. To encourage teams to participate in the training together, we offer discounts. The training fee includes tuition, course materials, CEU processing fees and continental breakfast, lunch and refreshments both days.

Training Conference AND Problem Solving Treatment Workshop (September 28-30)
Individual: $315 Advance (Before or on 8/28/09); $365 Late (AFTER 8/28/09)
Two: $265 per person Advance (Before or on 8/28/09)*; $315 per person Late (AFTER 8/28/09)*
Three or more: $235 per person Advance (Before or on 8/28/09)*; $285 per person Late (AFTER 8/28/09)*

Training Conference ONLY (September 28-29)
Individual: $255 Advance (Before or on 8/28/09); $305 Late (AFTER 8/28/09)
Two: $205 per person Advance (Before or on 8/28/09)*; $255 per person Late (AFTER 8/28/09)*
Three or more: $175 per person Advance (Before or on 8/28/09)*; $225 per person Late (AFTER 8/28/09)*

*To receive the group rate all registrants must work at the same organization, identify the organization on each registration, and submit registration forms in the same envelope (mail-in) or within 5 working days of the first electronically submitted registration.

Problem-Solving Treatment Workshop ONLY (September 30)

Refund Policy

If you register and then are unable to attend or send a substitute, you may obtain a refund of the fee less $50 for handling by submitting a written request.  In the unlikely event of course cancellation, registration fees are automatically refunded in full.  Costs incurred by the registrant, such as airline penalties, are the responsibility of the registrant.
